Owner or handler responsibility: I understand that I remain responsible for my dog at all times during fittings, rehearsals, backstage preparation, and event participation.*
Accuracy of information: I confirm that the information provided in this application is accurate and complete.*
Media consent acknowledgment: I understand that selected participants will be required to sign a waiver and media release covering photography, video, livestreaming, and promotional use.*
Selection acknowledgment: I understand that submitting an application does not guarantee selection, stage time, social media coverage, or inclusion in the final show.*
Communication consent: I agree that RexyWear may contact me by email, phone, or text regarding this application, meet-and-greets, fittings, rehearsals, and event participation.*
